To remove the old wax guard, insert the threaded end in the opening of the wax guard, twist and pull out the old guard. To insert the new filter, flip the tool and insert the new wax guard. They are packaged similarly with the installation/removal tool already attached to a new wax filter, I haven't had any issues with pulling the the tool out and having the wax filter fall off. Once inserted into the hearing aid receiver they stay put, no evidence of falling out or being loose. The hole count in the filter screen matches the original Phonak wax filters also.
