I got ebook printed copy.The page numbers of chapters in table of contents missing; and there is no index at the end of the book.What book does not have index?

Really simple and useful

I heard many good reviews about this book but honestly it didn't stand to my expectations. I mean it's a good book but not something that I can use thoroughly for my exam.
