how do you use a disk break piston tool?

Use one of the old brake pads and use the brake pad spreader to push the Piston back into the caliper so you can fit the new pads in

I just did the rear brakes on my Honda prelude, the backs do screw in.

No. It is wrapped in plastic on a cardboard backing.
