Honestly it doesn't matter since the overall thickness is so thin you'd need a micrometer to measure it. 22mm inner? yes. 22mm outer? also yes.

Q: Does this extender screw in to an existing base, or is it to replace a base?

it screws in as a replacement and two screws up on the sides allow for adjustment of the height of the 'candle'
