Does the blade stay open without hold?

Yes. This is a manual carton cutter! The blade will is exposed when the slider is pushed forward and will stay exposed until the slider is retracted :)! TEAM SLICE // San Jose, California

How do you lock the blade in position? This is a clear safety hazard as the blade is slipping all over the place and opened in my pocket cutting me.

I've owned 2. In one case the outer aluminum shield isn't snug against the orange ceramic casing holding the blade. I used pliers to squeeze the aluminum just enough to hold the blade in place. It worked even to slide the blade in and out holding it in place. I don't understand about the blade cutting you though. The whole idea of the blade being ceramic instead of metal is so that it DOESN'T cut you. The "Slice" has opened in my pocket as well but I was never hurt. I really like this product.

Must have if you order stuff from Amazon.

It works as intended, it cuts through boxes with incredible ease, and does not cut your finger. I mistakenly put my finger on the blade and pressed, nothing happened. Had it been a razor blade or a utility blade I would have 100% needed stitching. Blade never goes dull.
