Can this be mounted on a flat surface for use as a golf cart rear view mirror?

That's exactly what I used it for and it works great We have to have our carts inspected by the local Police and they are always impressed with this mirror Highly recommend it

Is this mirror convex in both directions (like a round mirror) or only horizontally?

Yes both directions. If I hold it at arms length I can see the floor and ceiling probably about 5 feet behind me and a more extreme view of the sides. Like mentioned very little unnatural distortion.

I have these on the side view mirror of my 2015 Toyota Tundra. It fits great.
