I see that is says "essential oil blend" , can you tell me if that includes patchouli oil?

I re-read the ingredients and that's the only thing it says "essential oil blend" so I cant tell you for sure. I can tell you I hate the smell of patchouli oil and can detect it pretty quickly. I don't smell the oil at all. I can't say with 100% certainty, especially with the other scents as I have only tried the original.

Has anyone experience staining on their white shirts from this deodorant?

I have not experienced any staining at all, I've been using this deodorant every day for many years. Sometimes sweat can cause stains on shirts (and remember, this product is a deodorant, not an antiperspirant) so you could potentially see some staining from your own sweat, but not from this product.
