Would my 9 and 11 year olds be able to play this? Do we need paper to keep track of health?

Absolutely. Our 6 and 9 year olds love it. Health wise there are little boards and counters that take care of tracking.

For these card sets, should you buy them in a particular order? Is this the “first set”?

This is the first set and best to buy it first. But I absolutely recommend buying the expansions. This is an awesome game and a family favourite. Tons of fun. If you get the full set then you'll have 12 decks to choose from or have up to 12 players. The big box has a compete storage and divide system for all 12 decks. It really is mayhem and a must have in your collection of games.
