Does the job.
Although a bit pricey, this was worth the money in terms of fulfilling the need. The holder was compatible with my Epson 4490. One trick that might be needed is to first do a scan with the standard 120 holder so that Epson Scan will remember the location of the scan area. The scan software is looking for a specific scan area on the glass, based on three tiny sync holes at the top of the standard adapters. Then the 126 holder should be placed in that same area, aligned with a straight edge of some kind on the side to level it; I used a plastic ruler. The adapter is made of partially flexible plastic. Both top and bottom parts are elongated rectangles, with the top snapping down onto the bottom, similar to standard holders. The top part is somewhat fragile; be careful with it. The holder is properly sized for the film. The bottom part is slightly thicker than standard holders, so the film is slightly more elevated from the glass. I didn't see any focus problems from that. The adapter gave me better results than putting the negative directly on the scanner surface with ANR glass over it. It would be possible to build one's own holder from thin cardboard with slots cut, or from poster paper with some kind of edge weights, but it's probably not worth the time. This holder helped me scan some old family photo negatives. Even though there weren't many of them, it was worth the money to mitigate the aggravation and possible Newton rings of putting the negatives directly on the glass surface. If it were longer and held more negatives at once, that would be better, but it might make the holder too fragile structurally. Overall, it does the job well and met my needs.

my longest strip is 4 negatives and it looks like there is room for 2 more
This comment refers to working with the Epson 700/750. All the other comments here contain everything you really need to know... but one thing isn't made entirely clear. So let me lay it out. This adaptor works WITH the 120 adaptor the scanner came with. This adaptor fits INTO a channel of the 120 adaptor. The 120 adaptor has TWO channels. If you have a lot of 127 to scan, you want to order TWO of these, so you can use both channels in a single scan run, doubling your efficiency from a single scan run. The manufacturer alludes to this obliquely by referring to the fact that they'll adjust shipping on orders of multiple units. One adaptor holds up to about 6 frames of the 1 5/8 x 1 1/4 version of 127. I'm estimating; my longest strip is 4 negatives and it looks like there is room for 2 more.

The holder to get if you have 127 film & an Epson V750/V700 scanner
Finding a film strip of 127 film may be rare, this holder will do the job to scan.

Great!
Let's put aside the fact that it got here wicked fast. I just used this for the first time in my Epson Perfection V750 and I am impressed. I just scanned a strip of 127 negatives shot back in 1973 using this adapter. The negatives fit perfectly in the adapter and the adapter worked with the 120 carrier. The scan? Great!
