You need to unscrew the tab a little and blow into the opening. When it is inflated, quickly tighten the tab. You will lose air in the process, so be very quick.

By mouth but I had to enlarge the hole with a drill as it was too hard to blow.

I have two of them. They both work excellent. There is a metal valve the screws in the end that can be tightened with your finger. Are use it on airplanes it has really helped the comfort of my ride
