No you can only wear it vertically

Q: Can you slide trauma shears behind the tourniquet?

I do on my vest. I actually cut the molle strap on the pouch and use a high speed gear tie. I would think with the button style secure on the strap the shears would undo it.

Q: Will it hold a SAM tourniquet?

I use them for a Gen 7 CAT (6 x 2 x 1.5 inches). Fits snug & secure
