My 3 yr old son used it and he was almost 40 lbs at the time. But he wore it without a shoe.

with the cast it should not be a problem but if they have one size smaller I would go with the smaller. My so was 4 y/o when he broke his leg and a size 11 toddler.

I can tell you its 3 5/8" WIDE 7 1/4" LONG if that helps

The back end of the shoe has the velcro. This will allow the shoe to accommodate multiple sizes. I envision that the small size would be fine for a size 10 shoe. My child was four years old when he needed it.
