Q: Where are both of the attachment points for the shoulder strap? I'm concerned about the balance point.

The top clip is located just below the carry handle and the bottom clip is located right above the mesh pocket. There are four D-rings and the shoulder strap can be switched on either the left or right side of the bag depending upon what shoulder you prefer to carry the bag across.

Q: Does a modern driver stay put in this without falling out while waking?

Yes. I use the bag at the driving range and on par 3 courses. I have carried it with a driver, fairway wood, 2 irons, a wedge, and a putter (and my putter is a long armlock model). 6 clubs is about the max you can comfortably fit in the bag, but I have had no problems with clubs falling out.

Q: Where are these made?

As there is no tag inside that says, "made in the USA" I can only assume China or somewhere outside the country.
