Hook failed
Used lead handle with alternative collar, tucked behind harness for quick access. Moments after using it to hold the dog while another passed, my dog disappeared into a bush and the lead handle never came back with the dog... It may have been my fault but given it was previously secure, can only assume it has fallen off.
