Q: Will this keep a cat warm if it's placed on a tiled floor? Does it insulate well enough to not need an interior pad or something underneath?

I would guess so. One of my cats goes as you would expect, the other lies on top flattening it into more of a cat bed. It is all wool so should be quite warm, and I would not think you would need any extra under it. I had to buy two of them because my 3 cats all liked it, but they I’ve around to different spots, so it is not being occupied 24-7.

Q: Why are the dimensions 16 inches x 16 x 10 in the written description of the large cave but on the picture it says 15 x 21 inches? Which is it?

It appears to be 16 x 16 x 10... UNLESS your kitty has discovered the fun of jumping right on top of it, crushing it down to a double walled snuggle bed. I live in San Diego, CA., where the temperature is so mild that the two cats do not use the cave as a cave, for the warmth. Either way, they are a very great addition to your cat's comfort.

Q: Is the XL grey size that’s more expensive bigger then rest? Diameter?

Yes, there is a big difference between each size. I own the medium and small and my cats fit in both with room to spare. If your cat isn’t super large, you could get away without the XL.
