Didn't know there was more than one size but none came with it.

Q: Do you have to install the soap dispenser and paper towel holder, or can you just install the backsplash without those things?

You do not need to install the soap dispenser and towel holder. They bolt on after the backsplash is installed.

Q: How many hand washes between water changes?

I don't know, but quite a few. It depends on how much water a person pumps with their foot

Q: How can we make yhis to have hot water?

It would be difficult. You would need a source of hot water and an external hot/cold mixer to adjust the temperature. If you can do this, it would still be awkward to use. A simple short-term solution would be to fill the source jug with warm water.
