UPDATE:I originally wrote this review the first of January 2023; it's now the middle of March 2023.I still haven't sought medical treatment for my back. Currently I'm on the fence as to whether it was a lumbar issue, or possibly a gluteus medius muscle strain/injury/pull?Whatever the issue, the key word in that sentence is 'was'. I'm not saying I'm back to 100%, but I'm now definitely somewhere in the 90+%. I still sleep with this thing every night, and I still wrap it a little lower than the product guide recommends.All the daytime back pain and difficulty bending down and sometimes even walking, is gone! Obviously I can't guarantee others will have my same results, but as I said below, I would buy this thing again in a heartbeat -- it has seriously been worth every penny!Cheers again!________So, I'm fairly certain I have an issue with one of my lumbar discs but have stubbornly not sought medical attention just yet. Sleeping had been a nightmare when trying to sleep flat on my back -- that's to say, I could lay on my back without issue or pain, but the moment I would try to get up, the lower back pain was almost unbearable. So I started side sleeping, which only helped a small amount.Yada yada, I was finally recommended this and picked it up on a whim...It is absolutely incredible. I probably look a bit ridiculous tying this thing around my waist at night, but no matter. I've been using it nearly two weeks now and my sleep has improved at least 70-80% or better. It recommends putting the product around the small of your back, but I go lower for my own personal comfort.Seriously, I don't typically leave Amazon product reviews, but this thing has worked beyond my expectations and I needed to show them some love. It's a simple design and idea and if there ever comes a day when I wear this one out, I'll for sure buy another.Cheers!

Quality stinks!
Edit Nov 16, 2012: Changing my review because I got a new McKenzie Night Roll to replace the one I got a few years ago. I do NOT recommend this product at all.This new model stinks. It has about half the heft of the original product and flattens to almost nothing, so there's no support.---------Rec'd this yesterday, used it last night. It stayed in place while I slept. The back support kept me comfortable. This morning, I got out of bed for the first time in years without struggling or pain.

Too thick, but works well
I've been sleeping on it for a week or so and have definitely noticed that my lower back pain is better, but my one complaint is that a 4" diameter is excessive. I'm able to fall asleep, but at some point in the night it gets uncomfortable and wakes me. I think if it was thinner, it would be less noticeable to sleep on. My other complain is the tie; I don't even bother with it because it's so difficult. I just sleep on it flat. The cover is nice because you can throw it in the wash. I would suggest getting the S/M, unless you are pretty overweight. The sizing was a little confusing.

Seems to have fixed my back
Maybe it was coincidence but from the first night I slept with this thing under my waist, my back pain was 95% better. I have degeneration at the L5-S1 level and suffer bouts of significant pain. I usually have to treat it with drugs and chiropractic but this latest round, I bought this roll and the very next day the pain was almost all gone. I'm going to keep using it.I subtracted a star because the cover isn't quite long enough but that's a minor thing. I got the largest size, I don't know if the cover works better on the shorter versions. I don't tie it around my waist but just lie it on the bed and sleep on it. If I'd known that I would have gotten a shorter one. I assume the foam diameter is the same for all the sizes.I seem able to roll over in my sleep and get it repositioned just fine. It is firm enough that I don't think it will flatten too much very soon. It's not the most comfy thing in the world but I have no trouble falling asleep on it, I get used to it and forget about it in a few minutes, and no back pain is well worth it.

Keeps the curve in your lower back while you sleep
I bought this after my PT recommended it.I don't tie it around my waist, just lay it flat on the bed, parallel to the headboard. Then get in bed, sit up and reach behind me to slip it under my lower back arch before laying down on it. It is very soft so it compresses to fit perfectly in the small of my back. Relieves pain as I sleep. Much more comfortable than the typical half moon bolsters that are too hard and too big. Only con: it would take way more patience than I have to get the tube pillow case on it, because it's too tight a fit. Haven't missed it though. Highly recommend this pillow.

Worked for me.
When I got up in the morning, I could not bend over and had burning pain down my right leg. Was almost impossible to even dress. I saw a video on YouTube (Bob & Brad) and they were showing possible things to do to help this kind of back pain. I did the stretching and it did help. I ended up buying this lumbar pillow. It was a big awkward to start, but after about a week, I had no more pain in the morning. I could actually enjoy my shower. Worked for me.

Too thick and too stiff.
I bought this thinking it would help with my back when I am in bed. Absolutely useless and a waste of 57 dollars. Too stiff and too thick. Not eligible for return so I am stuck with a useless product. Would not recommend it. It's like if I threw away 57 dollars. Unbelievable!

Seems far too thick to sleep on.
I'm using this to sleep on my back and side. In both cases it just seems too large. I feel like I'm completely sway-backed sleeping on my back with it, and on my side it's like I've rolled on to another person. Disappointed that this extremely firm roller (has very little give) isn't going to work for me.
