No, they do not come with bearings.

Can they be used outside and inside?

Not if the outside surface is at all rough and you want to have a pleasant outside skate

What size bearings fit these?

Bearing size depends on your axle, not the wheel itself! The outer dimensions are always the same for a skate bearing. It's the inner hole that's measured. If you've got 7mm axles, get 7mm bearings....if you've got 8mm axles, get 8mm bearings.

Will these fit on regular quad skate trucks? Is the inside flat?

They fit perfectly on my quad skates
