can it be used permanently ? connect it to the main water system of the house to measure regularly?

Yes it can be used permanently attached to a 3/4 hose faucet you dont intend to use , like a washer port or outside type hose faucet valve . I have these attached to the clean out/Flush out on cold water input supply side of a tank less water heater (I just leave the ball valve open instead of having port closed/capped.This allows you to get water pressure reading after water filters and have a diagnostic water pressure gauge if you should have problems with your tank less water heater issues of flow/pressure input.

Where are instructions? What is the significance of the red pointer vs. the black pointer and how long is the testing period?

No instructions. Just connect it to a spigot and turn the spigot on. The black needle will indicate the current pressure. Set the red pointer to where the black pointer is. The red will stay put to let you know if there has been a change in pressure. Example: turn on water, black needle goes to 70 psi. Set the red needle to where the black needle is. Perform any tests (i.e. turn off the water at the main valve from the meter with the spigot still in the on position). Check the gauge. Any movement indicates a leak in the system. The red gauge is your benchmark setting now. I used it to detect leaks in my water system. My meter is about 1/4 mile from the house. My wife could give me readings over the phone while I made adjustments upstream of the house.

Gauage filled with water, is this defective?

Yes. The gauge is not supposed to fill up with water. Mine remained dry after connecting to the line.
