there are 4 pretty good size shopping carts with 8 empty spaces everyone takes a list and a cart and they try to fill their list first

If the child has low cognitive abilities, you could play it as a matching game with the pictures visible instead of turning them over like you would if you were playing a memory game. You could also have the child find three pieces instead of all the pieces. It's a very cute and fun game. All of the children I play it with never get tired of it.

How big is each tile?

I’d say that the tiles are approximately 1 inch by 1 inch the lists and shipping carts are more like 5in by 3in.
