Q: If you don't recommend machine wash, what do you suggest as the process for cleaning blood out of this underwear?

Poor hydrogen peroxide on any blood spot. let sit for however long you’d like (even in the laundry basket until you have a load) and then wash... comes right out! bedsheets, bed toppers, panties, everything!..Been doing it since I was a kid and it works every time.

Q: Are these undies less full or totally full coverage? How wide is the actual elastic inside the waistband

The coverage/sizing is correct 💯. However I didn't make this purchase for extra panties.

Q: Have recently read that some similar products contain pfas. any guarentee these do not?

I really don't know if pfas are in this product but if you happen to buy them get a larger size than you would normally buy they do run small.
